Ethereal Item Bug

The defense bonus to ethereal items will increase every time.

This requires a code edit to fix (its a vanilla D2 bug), IIRC it has been posted here for 1.13c. The softcoded work-around doesn't quite work correctly and requires a lot of cubemain entries (MedianXL tried it at one stage, became easier to just not allow eth items to be cubed).
