The dissapearance ratio is controllable to a degree, as you can set the percentage chance of this to happen. However, it's still just a percentage chance, an RNG. Think of it this way: if you toss a gaming cube, you have only ~16,66% chances to hit 6, but it doesn't mean you won't succed on a first ...

Yes, you can replace them, but then you have to edit warps from previous levels. IE if you delete the cave level 2, you have to edit the cave level 1 so it doesnt contain an entrance to the level you deleted.

I don't know about pre 1.10, but this effect is present in anything since (at least) 1.10. It happens thanks to the invisible unit (monster) with BaalTaunt AI, which is fully configurable.

You are welcome. In case of minHp, maxHp and L-HP, remember about difficulty levels, as there are also (N) and (H) variations of these fields.

2253 is not an exponentation of 2. If missiles.txt would have a limit arround 2000, it would be 2048. Anyway, according to Myhrginoc, the limit of this file is most likely 32768.

You should check your missiles id's order, it seems you made a typo there.
