I downloaded the d2Elements Beta 3 file and it did not include the .ini file listed in the readme.rtf
Where can I get this file?

i guess you are looking for the "PlugY, The Survival Kit.ini" file. The file IS included, but for some reason it is not properly extracted by some zipping programs. i can tell you for sure 7Zip works, i am quite sure WinRar does, too.
in the next version of the beta, i will include the most recent version of PlugY, where the file was renamed iirc. that will solve this problem anyways. until then, try to either open the .zip file (not just extract) or download another packing program
